CHF Program
Our CHF program is a comprehensive approach to the treatment of chronic heart failure. We pride ourselves on the quality care provided to our clients and use the latest in wireless CHF telehealth equipment.
A Specially trained cardiac nurse will oversee your condition with you and the type of care and services you will need. We will help you monitor your condition, answer any questions or concerns you may have and give you information about your condition. Support for your and your family is just a phone call away.

Stroke Program
Our stroke program follows CJW's extensive stroke pilot program guidelines and has been developed to follow the patient from the hospital to the home.
Wound Healing Program
Advance Care has partnered with PolyRemedy, Inc and Doctor Stephen Crossland of the Retreat Doctors' Hospital Wound Healing Center and Chippenham Wound Healing Center in a one-of-a-kind study to help improve wound outcomes with the use of a new Personalized Wound Care System (PWS), a secure web-based application that collects and tracks information about a patients' wounds and dispenses custom Polyfit® Antimicrobial wound dressings based on specific care treatment plans that are tailored to each patient.
This exciting study has the potential of significantly impacting, if not revolutionizing, the quality of care imparted to patients with chronic wounds.
